****Things you will need to enroll your child: ****

**Child must turn 5 years of age by 10/1/16**

**Current immunization record**

(Signed personal/medical exemption)

(Including 5^th DTP, 4^th Polio, 2^nd MMR, 2 -Varicella)

**State Certified Birth. Certificate**

Children must have Birth Certificate and updated

immunization to enroll.

**Enrollments will not be taken without them
